Understanding a simple procedure claim form
When a respondent is served a simple procedure claim form, it contains:
    a copy of the ‘claim form’ (Form 3A). This will have been completed by the claimant and should detail all the issues relating to the claim, including why the claim should succeed;
    a blank ‘response form’ (Form 4A). This form should be completed by the respondent and returned to the court before the expiry of the date for response if they want to respond to the claim;
    a copy of the timetable for the claim;
    a blank ‘time to pay application’ (Form 5A). If the respondent admits the claim but wants time to pay, they should complete this form and return it with the ‘response form’ (Form 4A) before the last date for response.
