This part of the questionnaire relates to a number of different conditions, including mental illness, learning difficulties, the effects of head injuries, and autistic spectrum disorders. You are asked broad questions about how your disability or health condition affects your daily life. The questions are grouped into seven headings relating to the activities listed in Box D.4 . Note that the wording in the questionnaire is sometimes different from the wording in Box D.4 (in which we use the exact wording of the law).
