9. The capability for work questionnaire

9. The capability for work questionnaire
We describe here the capability for work questionnaire that applies to both universal credit and employment and support allowance: the WCA50. The WCA50 begins by asking for general personal details (your name, address, etc).
You are asked to provide details of your GP and the healthcare professional, carer, friend or relative who knows most about your disability, illness or health condition. You are then asked if you would give consent for your doctor, or any other professional treating you, to provide the DWP with information on your health condition. Without this consent, it is more likely that you will be assessed by a healthcare professional working on behalf of the DWP (see
14 below
). You are asked if you have cancer and are having, waiting for or recovering from chemotherapy, radiotherapy or other cancer treatments (in which case, you may not have complete all the questions in the questionnaire).
