10. Completing the questionnaire – eating and drinking
This part of the questionnaire covers two activities in the ‘limited capability for work-related activity assessment’ (activities 15 and 16 in Box D.5 ):
    ■conveying food or drink to the mouth; and
    ■chewing or swallowing food or drink.
Write down on the questionnaire if:
    ■you cannot get food or drink to your mouth without help from another person;
    ■you can manage without help, but need to stop repeatedly because of, for instance, poor coordination or poor grip, spasm, shaking or weakness;
    ■getting food or drink to your mouth would cause breathlessness or severe discomfort;
    ■you need regular prompting to get food or drink to your mouth, or you need physical assistance to do so because you have a severe mood or behaviour disorder;
    ■you cannot chew or swallow food or drink, or cannot do so without stopping, feeling breathless or feeling severe discomfort; or
    ■you cannot chew or swallow food or drink without repeatedly receiving regular prompt
