You can claim using claim-form V6, which you can get by ringing 0300 330 0013 or by downloading it from: gov.uk/vaccine-damage-payment/how-to-claim. If you are deaf and use British Sign Language, you may be able to claim using a video relay service (details are on the website).

Don’t delay claiming. If you have supporting medical evidence, send a copy with the claim, otherwise the Vaccine Damage Payments Unit will obtain medical evidence on your behalf. If the disabled person is aged under 16, the claim should be made by the parents or guardian.
