After you have provided the DWP with the fit note (see Chapter 10(9)), or details of it, you should be referred for a work capability assessment. Once you have been referred, you will normally be sent a form to complete: the ‘capability for work questionnaire’. You will not be sent a questionnaire, however, if the DWP considers that it already has enough information to decide that you have (or can be treated as having) a limited capability for work and a limited capability for work-related activity; the DWP will let you know if this is the case.
