‘Continuity of care’ (also known as portability) ensures that your needs continue to be met if you move to another local authority area to live. The authority in the area you are moving to must, when you advise them of your proposed move, carry out an assessment before you move and take into account your current care and support plan (see 3 above ). It must also provide a written explanation if it assesses you as having different needs to those identified in your current plan.

If the local authority in the area you are moving to has not carried out the assessment before you move, it must continue to meet the care and support needs that were being met by the original authority until it has carried out its own assessment.
