21. How your claim is assessed
Your claim will be assessed using the completed Part 2 of the application and any supporting information. Supporting information could include:
    ■occupational therapy or physiotherapy reports;
    ■social work reports or assessments;
    ■care plans or assessments;
    ■treatment plans; and
    ■documents that show your local authority has made changes to your home (eg by provided a stairlift, ramp or accessible shower).
You can provide such supporting information to Social Security Scotland yourself. You can also include accounts from family or friends or a diary of your day-to-day needs (see Chapter 5(16) ). Do not delay completing Part 2 of the application if you are waiting for the information to be sent to you; you can let Social Security Scotland know that you have asked for it and forward it to them once it does arrive.
If you do not provide them with any supporting information, Social Security Scotland can gather it themselves; you will be asked to give them consent to
