7. Discharged before you’re ready?
If acute nursing care in hospital is no longer essential for you, the hospital will clearly wish to discharge you. It may be impossible for you to return home without support services in place. Before being discharged, your care needs should have been assessed. Don’t agree to a discharge unless you are happy with the arrangements for continuing care and support as set out in your care and support plan (see
Chapter 38(3)). Don’t agree to move to a care home
unless you are absolutely clear about how, and for how long, the full cost will be met.
