A DWP decision maker looks at the information you have provided with your claim for ESA or universal credit to see, without having to make further enquiries, if there is evidence that you have a limited capability for work and for work-related activity. They should also see if there is evidence that you can be treated as having a limited capability for work and for work-related activity (see 3 and 6 above). If the decision maker considers they do not have such evidence, they will send you a questionnaire to complete: the ‘capability for work questionnaire’.
